Membership Application

To become a Madison Originals member, please fill out the form below and press the “Submit” button. Please email board@madisonoriginals.org if you have problems, or need assistance.
The following information will be used for the MadisonOriginals.org website.
Membership Fees: Restaurants – $1800 in gift certificates sold at 40% discount in an online quarterly auction, Allied Members – $1600 in gift certificates sold at a 40% discount in an online quarterly auction; Associate members select a level below, and mail payment to: Madison Originals, Inc. PO Box 259501 Madison, WI 53725-9501.
